I Am Heart Healthy
A community health advocacy program of Community Revitalization Alliance, Inc. (CReA), in partnership with the Association of Black Cardiologists (ABC). Through trained Community Health Workers, we bring heart-health education, screenings, healthy lifestyle support, and connections to healthcare resources directly into trusted community settings. Participants learn to understand their health numbers, reduce risk factors such as high blood pressure, diabetes, high cholesterol, and obesity, and take practical steps to prevent heart disease and stroke. Our goal is simple: empower residents with the knowledge, tools, and support to take charge of their heart health and live healthier lives.
